Care Management, also known as “Tailored Care Management,” is a program provisioned by North Carolina Medicaid. Thus, only North Carolina Medicaid recipients are eligible for this program. Our care management service consists of a team of professionals guided by a Care Manager who support individuals living with behavioral health issues, substance use disorder, intellectual and developmental disabilities and/or traumatic brain injuries.

Our Care Management team develops a plan that is “tailored” to the needs of the patient and incorporates all individuals such as mental health and medical professionals, family members, and community services that will assist the patient with achieving the highest quality of care to live their best possible life!

Through tailored care management patients will have a single designated care manager supported by a multidisciplinary care team to provide whole-person care management that addresses all of their needs including physical health, behavioral health, I/DD, traumatic brain injuries (TBI), pharmacy, long-term services and supports (LTSS) and unmet health-related resource needs.

Our partners also have the ability to provide many services via telehealth, which makes life and service provision more for patients. We will begin implementing out Tailored Care Management Services beginning December 1^st, 2023 and look forward to assisting patients with their needs.
